Patient Forum

The next Patient Forum will be held on Tuesday 28th February 2012 at 6.30pm at The Law Medical Group Practice, 9-11 Wrottesley Road, London, NW10 5UY. The main focus of the forum will be to discuss the results of the practice's recent patient survey. If you would like to attend, please complete an attendance reply slip at reception or which can be printed HERE and posted for the attention of the Practice Secretary. Please be advised that places are limited.

We are aiming to proactively engage patients in helping to shape our service and we would like to hear your views. If you are interested in becoming a member, please fill out a form that can be found at reception, from your GP or Nurse and also can be printed here.
 

Changes to Paediatrics Services at Central Middlesex Hospital

The service for seriously ill children at Central Middlesex Hospital is transferring to Northwick Park from 25th November 2011. More information about the changes can be found here.

PCT Practice List Validation Exercise

Have you recently moved house? If so, and you have not told the surgery then you risk being automatically removed from our list by NHS Brent. Please remember to:

• Reply to letters from NHS Brent that ask you whether you are still living at your address of if you are registered with your GP.
• Tell the surgery if you move house.
• Register with a GP when you move into the area.

If you have been removed from your GP's list, please re-register without delay. If you would like to complain about this issue please contact Mr Tony Menzies, Interim Project Manager, c/o PALS, Wembley Centre for Health & Care, 116 Chaplin Road, Wembley, Middlesex, HA0 4UZ.
