Join the Launch of the North West London Black African and Caribbean In-Reach Model – December 2025
We are delighted to invite you to the launch of the North West London Black African and Caribbean In-Reach Model, taking place on Saturday 6th December, from 11am to 4pm at Brent Civic Centre.
This special event marks the beginning of an exciting new approach led by the North West London Integrated Care Board (NWL ICB) designed to bring health services directly into trusted community spaces.

About the In-Reach Model
The In-Reach Model focuses on working with communities, not just for them. It aims to:
- Build trust between health professionals and local residents
- Improve access to health and wellbeing services
- Co-produce culturally appropriate care that reflects community needs
- Strengthen collaboration between local organisations, NHS partners, and Integrated Neighbourhood Teams (INTs)
Initially, the model will focus on African and Caribbean communities, helping to shape a more inclusive and equitable health system across North West London.

The event will include:
- Free health checks and wellbeing activities
- Delicious food and refreshments
- Community conversations and networking opportunities
- The official launch of five new in-reach hubs across five North West London boroughs
