How To Book An Appointment
Through your GP practice:
- Ask a member of the reception or clinical team for a referral to Bromley by Bow Centre / social prescribing service.
- A member of their team will then contact you to arrange an appointment.
Eligibility:
- Adults registered with participating local GP practices who would benefit from extra social, emotional or practical support. You do not need an urgent medical problem to be referred.
Patient aftercare and follow‑up
You will usually have a series of follow-up appointments (in person, by phone or online) to review your goals and progress.
Your link worker or support worker will:
- Check how you are getting on with any services, groups or activities you have been connected with
- Adjust your plan if your needs or circumstances change
- Provide further signposting if new issues arise
With your consent, they may update your GP so your medical team understands the wider support you are receiving.
