Our Service’s Include

We provide compassionate palliative care and coordinate with specialists, including the Brent Child Health Hub, to support patients and families.

  • Compassionate palliative care for patients and families.
  • Support for children and young people through the Brent Child Health Hub.
  • Coordination with GPs, paediatricians, and wider multidisciplinary teams (MDT).
  • Guidance on pain management, emotional support, and family care.

What to Expect During an Appointment

You will meet face-to-face with a GP and pediatrician, and your case may be discussed in a virtual multidisciplinary team meeting to plan the right care.

  • Face-to-face consultation with a GP and/or pediatrician at a local GP surgery.
  • Discussion of your concerns and needs.
  • A virtual MDT meeting with wider professionals if required.
  • A personalised care plan tailored to your child or family’s situation

Patient Aftercare and Follow-Up

Our team offers ongoing support, regular reviews, and guidance to ensure your child and family receive the care you need.

  • Ongoing support from our Care Navigator, Meghan and GPs.
  • Regular reviews to monitor progress and adapt care as needed.
  • Access to specialist services via the Brent Child Health Hub
  • Guidance for families on additional resources and support networks
