Patients can access AJM’s services through self-referral or a referral from a qualified healthcare professional. If you are referred by a healthcare professional, you will receive a letter or phone call informing you of your eligibility for a wheelchair assessment.
What is a Wheelchair Assessment?
A wheelchair assessment is conducted to evaluate your clinical needs and determine the most suitable wheelchair prescription for you.
Where Do Assessments Take Place?
Your assessment will be conducted at one of our designated wheelchair clinics, located in:
- Park Royal
- Wembley
- Barnet
- St. Charles Hospital, West London
In some cases, assessments may also take place in an alternative setting, such as a school or healthcare facility.
Home assessments are only available under specific circumstances and will be determined following a triage assessment by our clinical team.
