We are now registering patients from a wider area of Brent and also the South Side of the Harrow Road within the Kensal Green ward.
How to Join Our Sites
If you wish to register with the practice please print and complete the following 2 forms: GMS1 Form and The Health Questionnaire. The GMS1 you will need this to fill in your personal details. Please complete all sections on the form that are relevant to you. Check both sides of the form.
The practice health questionnaire, you will need to complete this as we need to know as much health information about you as possible to allow us to give you the best possible care. If any of the persons being registered are under 5 years of age we need to have all their immunisation history.
Please make sure that you add a current telephone number or mobile number on the form in the event that we may need to get in touch with you about your registration. After completing the forms, you can email it to our generic email address [email protected] with a copy of your photo ID, or place in a sealed envelope and posted through the practice letter box, or just post it to our practice.
