We have a restricted geographical catchment area as this enable us to offer a more efficient service to all our patients.
This Clinic provides a service for all newly registered patients. A trained Health Care Assistant will provide a health check and discuss your medical history and lifestyle with you, as well as other general information regarding your health. You will need to bring a urine sample for this appointment.
All new patients who fail to attend will not be registered with the practice until a health check clinic has been attended.
Identification is required to enable the process. The following documents are acceptable:
- 2 utility bills
- Tenancy agreement
As well as a
- Passport
- NHS Medical Card
- Previous GP details.
People who have never been registered with a doctor in the United Kingdom will need additional documents such as proof of work in the United Kingdom or if studying proof of registration with a teaching establishment in the United Kingdom.
